In collaboration with the Saskatchewan Health Authority (SHA), the Community Paramedicine Model in Moose Jaw is a connected care model, utilizing Community Paramedics to provide medical support and treatment as well as resources in a community-based setting for individuals who are experiencing mental health illness and substance use disorder.

Services include:

  • Clinical Assessments & Diagnostics (12-Lead ECG)
  • Physician, Nurse Practitioner and Psychiatry Consults
  • First Dose Antibiotic Administration
  • Mental Health and Addiction Supports (Home Detox Treatment/Support/Medical Oversight)
  • Emergency Department Supports
  • Administration of Intravenous Fluids/Medications & Medication Assistance
  • Vital Sign Monitoring
  • Wellness Checks/Clinics
  • Nasopharyngeal/Oropharyngeal Swabs
  • Pain and Symptom Management
  • Clinical Illness/Disease Surveillance
  • Phlebotomy
  • Wound Care and Dressing Changes

Registered Nurse hired through Medavie Health Services.
The Registered Nurse plays a key role in promoting health and preventing illness and will supplement existing Primary Health Care services and will play a crucial role in identifying health needs and risks, develop and implement health programs, provide direct patient care and collaborate with other healthcare professionals to improve access and health to the youth of the community.

As part of the Mobile Integrated Health Team, this position works with the YMCA Homebase Staff responds to a variety of patient populations including youth in need of mental health and addiction services and support, physical health services, and coordination of care and supports in the community. The Registered Nurse works: collaboratively with other interdisciplinary service providers such as, but not limited to, Community Mental Health and Addictions, Primary Health Care, Psychiatry, Paramedic Services, justice, victims' services, child and family services, and police services and other service providers.

Hours:
Tuesday's/Thursday's; 3:00 PM - 7:00 PM
Third Saturday of each Month; 1200 PM - 3:00 PM
